Registered Nurse

Positions available throughout Southwestern Pennsylvania

Summary of Duties

Our Registered Nurses provide direct patient care; teach and supervise others in aspects of patient/family care, evaluate patient/family needs, plan patient care and refer patients for other services as needed.

Essential duties and responsibilities include the following:

  • Obtain patient medical history, particularly as it relates to the patient’s present condition, from the patient or family member.
  • Perform physical assessments to identify patient needs.
  • Evaluate the patient’s family dynamics, home situation and environment. Evaluate the patient’s condition to determine what type and frequency of services are required and obtains physician’s orders for same.
  • Establish maintain and/or revise patient plans of care.
  • Evaluate the effectiveness of patient plans and make revisions based on patient’s changing needs.
  • Inform physician of changes in the patient’s condition, plan of care and/or frequency and type of services needed.
  • Consult and coordinate care with other active caregivers.
  • Carry out other duties that may be assigned.

Supervisory Responsibilities

Instruct and supervise Licensed Practical Nurses and Aides in specific care needed to meet the goals of care.

Education and/or Experience

  • Graduate of an approved school of nursing; and one year of experience as a Registered Nurse.
  • Certificates, Licenses, Registrations.
  • Must possess a current license to practice in the State as a registered professional nurse.
  • Must maintain a current CPR certification.


Licensed Practical Nurse

Summary of Duties

Provide direct nursing care to patients. Educate patients and their families on aspects of the patient’s care, disease process and medications.

Essential duties and responsibilities include the following:

  • Provide nursing services to patients under the direction of a registered nurse.
  • Identify and report to the coordinating nurse and physician health needs of patients and families through the observation of their physical, social and emotional needs.
  • Administer nursing care to patients according to the care plan.
  • Contribute to the development, implementation and evaluation of care plans through conferences with the coordinating nurse.
  • Report and record patient condition and care.
  • Administer medications as per physician’s orders.
  • Carry out other duties that may be assigned.

Education and/or Experience

  • Graduate of an approved school of practical nursing and one year experience as a licensed practical nurse.
  • Certificates, Licenses, Registrations.
  • Must possess a current license to practice in the State as a licensed practical nurse.
  • Must maintain a current CPR certification.


Physical Therapist

Essential duties of a Physical Therapist:

  • Assess and screen patients’ daily living/working skills and develop a therapeutic retraining program with measurable objectives.
  • Administer and interpret diagnostic and prognostic tests of function to patients in their homes or other settings to assist in evaluating the patient’s level of function.
  • Confer with the patients’ physicians and other healthcare team members and participate in development/revision of the plan of care.
  • Provide therapeutic treatment and instruction to patients in accordance with physician orders to improve/restore strength, coordination, range-of-motion and function or teach compensation measures.
  • Review and expand retraining programs in a manner consistent with the behavioral goals of patient and within the guidelines of the rehabilitation program as a whole.
  • Carry out other duties that may be assigned.

Qualifications

  • Doctorate or Master’s Degree in Physical Therapy; and a minimum of one year experience as a Physical Therapist.
  • Must possess a current license to practice in the State as a licensed Physical Therapist.
  • Must maintain a current CPR certification.


Occupational Therapist

Certified Nursing Assistant

As a Certified Nursing Assistant your essential duties are as follows:

    • Assist patients with personal care needs: bathing, elimination needs, transferring, dressing /undressing, and ambulation together with any household tasks (e.g., making bed, changing linens, laundry, light housekeeping, etc.) essential to the patient’s health in accordance with the personal care plan.
    • Plan/prepare nutritious meals and assist in feeding patients as directed.

Document services provided in an accurate and timely manner in accordance with Company policy and personal care plan.

  • Maintain patient confidentiality at all times.
  • Measure and record patient’s temperature, pulse, respiration and blood pressure, when ordered and/or directed by the RN Supervisor and personal care plan.
  • Carry out other duties that may be assigned.

Qualifications

  • Certified Nursing Assistant must have successfully completed a state approved certified nursing assistant program.
  • Minimum of one year of experience.
  • In order to maintain certification, certified nursing assistants are required to attend a minimum of 12 hours continuing education programs (These programs are offered through Harmony).


Personal Care Aide

As a Personal Care Aide, your essential duties are as follows:

  • Assist patients with personal care needs: bathing, elimination needs, transferring, dressing /undressing and ambulation together with any household tasks, (making bed, changing linens, laundry, light housekeeping, etc.) essential to the patient’s health in accordance with the personal care plan.
  • Assist patients with homemaking, including meal planning and preparation.
  • Shop for groceries and essential household or medical supplies.
  • Accompany patient to medical appointments when approved by the Registered Nurse Supervisor.
  • May be asked to assist with other duties.
  • Document care daily and submit all required paperwork weekly

Certificates/Qualifications

  • Must have successfully completed a Personal Care Aide training program; or have successfully passed an equivalency exam and provide written evidence of such completion.
  • Previous experience in a home environment desired.
  • Must be mature, emotionally stable and must display a sympathetic attitude toward providing services for patients at home who are medical problems.
